What is primary injury?
Immediate effect of trauma which includes forces of compression, contusion and shear injury to the spinal cord
What is a secondary injury?
Progressive mechanism of cord injury
What is the initial evaluation of SCI?
ABCD - airway, breathing, circulation and disability
What are sx of SCI?
Head injury
Unconscious or confused
Complaints of spinal pain
Weakness or loss of sensation
How do we treat CV complications of SCI?
Neurogenic shock:
IV fluids, transfusion, vasopressors
How do we treat respiratory complications of SCI?
Frequent suctioning
What are treatments of SCI?
Glucocorticoids (MP)
GM-1 ganglioside
How does GM-1 ganglioside work?
Protect against additional nerve cell death after spinal cord injury and also stimulates nerve-fiber growth and repair
What is given for the anticipation of bradycardia?
Atropine
What drug is avoided in SCI?
Succinylcholine after the first 48 hours
How do we treat pain in SCI patients?
PCA
How do we prevent VTEs in SCI?
SCDs
