with spina bifida around the segmental level, person will be
impaired, 2-3 segments below. where at some point below where be complete SCI signs.
anencephaly
failure of closure of rostral end of the neural tube. if towards the head- baby dies. if towards the butt - spina bifida
Myelomeningocele
defective closure of the neural tube in the vetebral column
hydrocephalus
Increased spinal fluid in the brain
syringomyelia
cyst in the spinal cord. treatment is to drain the cyst to have less pressure around that area. how do we know changes in sensory, strength and spasticity.
tethered cord
scarring which attaches spinal cord which makes it so that you cannot glide your nerves.
all children with spina bifida have a
tethered cord at some point but not all have the symptoms.
T5-T12 prognosis for spina bifida
weak abdominals and paraspinals; absent lower extremities. Primarily manual wheelchair movers.
L1
limited walking.. heavily compensated because they don’t have glutes or quads. If you cannot activate your quads you will need a need KAFO
L3-L4
gain quads, but still dont have glutes to keep trunk up. Still limited to household walking. If you cannot activate your quads you will need a need KAFO
Sacral levels
gain glut and gastroc. Will walk inefficiently but will be able to walk in the community.
Is the prognosis better or worse for babies who get the prenatal surgery
better
Babies that are born before 38 weeks AGA will be defined
premature (all babies with spina bifida will be premature because they are taken out with a C-section at 36 weeks AGA)
Pts with spina bifida tend to have scoliosis due to their.
poor sitting posture. PTs must track this because no other clinicians do.
Early intervention for which transitional skill is super important for kids with spina bifida
wheelchair to floor
