Spijker, Wentink-van de Laar, Huson & Schene (2019). Practice guidelines and standards of care in mental health care. Flashcards
The Netherlands has taken significant steps to improve the quality of mental health care through the development and implementation of multidisciplinary care standards. Initially spearheaded by the Dutch Foundation for Quality Development (NKO) and later managed by Akwa GGZ, this initiative addresses gaps in previous monodisciplinary guidelines by integrating scientific evidence, clinical expertise, and patient perspectives into cohesive, nationally accepted standards of care.

Between 2014 and 2018, NKO developed over 40 practice guidelines, including disorder-specific standards (e.g., for depression) and generic standards (e.g., self-management and societal participation). These standards reflect a patient-centered approach and outline expectations across the entire care continuum, from prevention to recovery and reintegration. The development process was inclusive, involving stakeholders such as patients, families, professionals, and insurers, with efforts guided by surveys, focus groups, and consensus meetings. Key priorities included addressing persistent depression, emphasizing non-pharmaceutical treatments, incorporating arts therapies, and creating relapse prevention plans.

One of the critical innovations was the integration of scientific evidence, clinical expertise, and experiential knowledge. While methods like GRADE evaluate scientific evidence, no universally accepted framework exists for combining different knowledge domains. Practical examples, such as including arts therapies and psychoanalytic psychotherapy despite limited scientific support, highlight the challenges and the need for consensus-driven decision-making to balance diverse perspectives effectively.

Akwa GGZ has taken a structured approach to implementing these care standards nationwide, focusing on practicality and accessibility:
- Resource Development: Online databases, user-friendly versions of standards, and implementation toolkits were created to support professionals and patients.
- Monitoring and Support: Implementation progress is tracked, with professional organizations responsible for actual adoption.
- Incentives: Insurers play a key role in promoting standards as benchmarks for quality care.
the care standards emphasize:
- Recovery-focused care, addressing reintegration, empowerment, and peer support.
- Matched care tailored to patient preferences and severity of illness.
- Limited use of antidepressants and a preference for non-pharmaceutical treatments.
- A multidisciplinary approach that includes the social network and emphasizes participation in society.
future challenges
Despite the program’s comprehensive scope, challenges remain. These include understanding the financial implications, creating sustainable processes for updating standards, and evaluating whether adherence to these standards improves patient outcomes and care quality.
The Dutch initiative to develop and implement care standards marks a transformative effort to align mental health care with evidence-based, patient-centered practices. While successful in addressing earlier guideline limitations and fostering stakeholder collaboration, the program’s ultimate success depends on overcoming challenges related to implementation, funding, and measurable outcomes. If these goals are achieved, the Dutch model could serve as a global exemplar for improving mental health care systems.
