Normal peaks in Hunter’s angle
Cho < Cr < NAA (N-acetyl aspartate)
PPM for Cr and NAA
Cr at 3 PPM, NAA at 2 PPM
Decreased NAA, increased lactate
MELAS
Elevated NAA
Canavan disease; NAA is normally the highest peak, but higher than normal
Elevated alanine
meningioma
Elevated glutamine (Glx)
hepatic encephalopathy; Glx is a “ramp” up to the NAA peak
Decreased Cho, decreased NAA, elevated lactate
radiation necrosis
Elevated Cho, decreased NAA, elevated lactate
high-grade malignancy (e.g. GBM)
Elevated myo-inositol (mI)
Down’s or Alzheimer’s; mI peak is before Cho
Significance of Cho
Cho is a non-specific marker for cell turnover (tumor, infarction, or inflammation)
Significance of lactate
lactate is seen with aerobic metabolism (necrotic tumors, radiation necrosis, abscess); peak at 1.25 PPM
