Spastic Dysarthria Intervention Flashcards
Respiratory intervention
Relax neck, shoulders and upper chest.
Transfer respiratory effort to abdominal level during breathing.
Inspiratory checking- deep breath in and out slowly to regulate flow of air and volume of air.
breathy sighs- to establish easy airflow.
Adjust posture.
U tube manometer.
abdominal support by pushing on abdominal muscles during exhalation.
Girdling abs with elastic bandage- prosthetic.
the aim for respiration is..
to produce better, more controlled airflow.
What is the aim for phonation intervention?
there’s laryngeal hyper-adduction, stained/hoarse voice.
Aim is to establish a more relaxed less strained voice.
Name intervention techniques for phonation.
General body, head and neck relaxation exercises.
Vocal exercises to decrease laryngeal tension.
EMG biofeedback to reduce laryngeal tension
other biofeedback- Visipitch :) provides instant visual feedback on computer for pitch variation.
Whats important in intervention?
Train them to self monitor :)
Treatment of articulation- what is the aim?
work on speed, rate and force, isn’t appropriate.
Aim- gentle approximisation of consonants and emphasize clear vowel production with minimum tension.
What’s a good way to do therapy for gentle approximasation?
Work in hierarchy- CV, CVC etc. etc.
What also helps reduce tension for articulation?
EMG biofeedback.
What prosthetic intervention is used for articulation?
Jaw sling- maintain jaw closure
Bite blocks- stabilise jaw and force them to make lip and tongue movements.
Resonance intervention- aim
Improve antagonistic-agonistic velopharyngeal muscle activity
name techniques for resonance.
slow and irregular brushing and stroking
desensitisation to hyper-reflexia of jaw movement
What prosthesis is useful for resonance in spastic dysarthia?
Palatal lift :)
Help compensate for reduced or incoordinated velopharyngeal muscles.
Prosody intervention
monotonous pitch - use visipitch or help increase self-montoring when reading passages etc.
Pacing board to regulate rhythm- help slow to reduce spasticity.
