Cryptococcus HINTS

A

In environment, but in pigeon droppings and eucalyptus trees, can cause cryptococcal meningitis
Diagnosis- Blood test for cryptococcal antigen. Available in serum and CSF.
Stains on INDIA ink, ink will not penetrate capsul, which is pathomnumonic.

Treat with Amphotericin B or High dose fluconazole.

What patient populaiton do you see aspergillus in

A

In immunocompromized patients or in patients who are on chronic steroids (COPD exacerbation, Airway acting up- may be aspergillus.).

To test for this: do Aspergillus test in blood or 1,3 Beta glucan, culture, to test for it.

Aspergellus in airway/broncahl infection- chest x-ray clear, needs to be treated with Steroids- they have mold in their airway- IgE is high. Have bronchosis aspergillosis.
