sop 03.05.01 Facility Maintenance Repair Requests
Emergency Repair Requests: Involve potential injury, loss of life, property damage that is immenent or already occuring:
no hot water
broken water pipe with uncontrolled flow
loss of electrical power
unable to manually close /open bay door
loss of heat/air con
clogged toilet if no alternative, water damage
Non emerg: repairs that can wait
broken faucet with water able to be controlled
broken appliance
flag replacement
Contact facilities through facility Wokr Order System or fire.facilities.gov
Emergency repair during normal hours (m-f 0600-1630) call facilities coordinator in resource management section
other hours, call number listed in sop
Sop 02.04.03 Fitness or duty
-Fill out FRD 025 a supervisors statement for request fitness for duty evaluation
For minor injuies/skin: contact OHC…must have DR fill out a FRD 158
- If under influence of drugs: place employee on admin leave, immediately relieve employee from assigned duties and responsibilities, keep employee at work location, no food or drink, notify BC and DC, if determined fill out frd 025, notify the professional standards office to have employee tested for drugs, give employee forms that explains the testing steps, employee transported to selected location for testing asap
-for injuries/sick: notify chief officer (BC, DC), relieve employee, fill out FRD 025 for why relieved, notify battalion chief of health and wellness, employee must have FRD 158 before returning to work
Sop 08.06.03 fire prevention system activation
What are the 4 definitions of fire alarms?
Impairment- is entirely/portion of is OOS…may not function properly if fire
Malicious- unwanted activation of an alarm initiating device caused by a person with malice
Nuisance alarm- unwanted activation of a signaling system or alarm initiating device in response to a condition that is jot the result if a potentially hazardous condition (aka dust)
Unwanted alarm- any alarm that occurs that is not the result of a potentially hazardous condition
If OIC cannot determine that an unwanted alarm has occurred, then can assume system functioned properly
Any incident that required a fire protection system or portion if to be OOS the considered impaired
Notify the fire inspections section or on call fire investigator/on call fire investigator prior to leaving scene for any impairments including:
Occupancy name
Contact name and #
Incident info
malicious or Nuisance alarm or mechanical damage to system found, “False alarm module” on imagetrend
08.04.01 building Addreses improperly displayed
Fill out the inspections violation notification e form
If address not over main entrance
Sop 08.04.01 requesting a fire investigator
When:
Origin of fire undetermined, suspicious…etc
Fire/explosion/environmental crime that involve desth/injury
Burns from death, direct flame that requires med attention except electrocution, fireworks, chemical burns
Explosives
Fires involving county property
Juvenile fire starter
Hazardous Events that may create media interest
Malicious incidents involving fire protection equipment
SOP 01.03.01 Vehicle Accidents
EMployees only talk to PD, supervisor, other FRD officials, RMD, or reps of FC claims service company
Obtain full name, address, insurance co, policy number of driver, and names of witness
Can provide those involved with” Inc #, Email for Risk Management, Refer to property damage and injury claim reporting
notify Duty App if needed
Complete a Vehicle accident, FRD 072
Everyone fill out FRD 317
Submit M5 if drivable
SOP 020905
Arrests and Civis Court
24 hours to report to supervior
fill out FRD 035 record of employee arrest, send to Profesional Standards Office
Refer to EAP
SOP 02.00.14 Workplace Violence
All employees must report violence to supervisor
Supervisors will investigate and report up the chain
employees with restraining orders (FRD 085 protective order) must present to the deputy chief
Supervisors, if possible, must attempt to diffuse situation before/during incident
Contact Professional Standards Officer if need be
Fill out Workplace Violence Incident Report Form (FIN 03), send to Human resources Manager through chain of command
SOP 02.00.06 Oral and Written Reprimands
Oral: Form FRD-133
employee has right to review records, and has right to attach personal statement to the oral reprimand form
OR kept for 1 year
WR: in a formal letter form, must list previous offenses with progressive discipline
SOP 02.00.03 Tardiness
Oral rep LWOP for time/AL 2nd time: Written, LWOP/AL 3rd Proposed LWOP x2days
Unathorized abscene: Fail to notify one hour after reporting time written rep, LWOP More than one hour: proposed suspension 2days LWOP 2nd time: proposed dismissal, LWOP
SOP 02.00.02 Discrimination, Harassment..etc
Reports of discrimination/harrassment/retaliation can be made to:
supervisor, FC or AC, PSO, OHREP…OHREP when in doubt
Report to Immediate supervisor and OHREP
Can make complaint to Equal Opportunity COmmision
