What is aspiration done for
- blood sample
- venous blood
- eg FBC, U and E , haematinitcs , glucose
When is fine needle aspiration biopsy carried out
- aspiration of cells from solid lesions
- neck swellings , salivary gland lesions
- cytology
What is Excisional biopsy
- removal of all clinically abnormal tissue
- fairly confident of provisional diagnosis
- usually benign lesions eg. fibrous overgrowths, denture hyperplasia , mucoceles
- discrete lesions
What is incisional biopsy
- representive tissue sample
-larger lesions
-uncertain diagnosis
What is punch biopsy
- type of incisional biopsy
- hollow trephine 4, 6 or 8mm diameter
- removes core of tissues
- minimal damage
- may not require suture or only minimal number of sutures
How to select an area to biopsy
- large
-representative
-refer suspicious lesions
What should a sample be placed in
10% formalin
Don’t use gauze as distorts sample
Examples of injuries to biopsies
Crush injury
Tear injury
What is fibrous epulis
- swelling from gingivae
-hyperplasticity response to irritation
-smooth surface , round swelling
-pink and pedunculated
-Excisional biopsy
-remove source of irritation
What is fibrous overgrowth
- fibroepithelial polyp
-frictional irritation of trauma
-semi pedunculated or sessile
-pink
-smooth surface
-buccal mucosa or inner lip
-surgical excision
What is giant cell epulis
- periphehral giant cell granuloma
- multinucleated giant cells in vascular stroma
- teenagers, anterior regions of mouth
- deep red or purple, broad base
- need to x-ray to ensure not centrally originating (would appear as radiolucency)
- surgical exision with curettage of base
What is haemangioma
- developmental overgrowth
-exophytic
-blue in colour
-pressure will cause loss in colour
-surgical removal or cryotherapy
What is lipoma
- benign neoplasm of fat
- soft swelling
- pale yellow
- sessile
- exision
What is pregnancy epulis
- histologically same as pyogenic granuloma
- may be related to calculus and bleed easily
- hormonal changes enhance responce to tissue irritation
- small lesions may not require exision and may regress after birth of baby
- larger lesions should be excised
What is pyogenic granuloma
- arises from failure of normal healing
-overgrowth of granulation tissue
-may be related to extraction sockets or traumatic soft tissue injuries
-red in colour
-surgical excision
What is squamous cell papilloma
-benign neoplasm
-usually pedunculated
-white
-cauliflower
-looks like warts
What is denture hyperplasia
- poor fitting denture
-hyperplasticity reaction
-roll of excess tissue on outer aspect of denture flange or between flange and alveolar ridge
-trim denture
What is leaf fibrous
-chronic irritation from denture
-would be round if not covered by denture but becomes flattened
-pedunculated
What is mucocele
- mucuous extravasation cyst
-common
-damage to minor gland duct
-recurrent
-soft blueish swelling fluid filled
What is ranula
FOM mucocele
what is SCC
- red or white , lump
-non healing
-ulcer, rolled margin , induration
