- end fistula
- epithelialized track
- origin (lateral duodenum, stomach, and ileum)
- adjacent large abscess
- intrinsic intestinal disease (Crohn, radiation enteritis, cancer)
- distal bowel obstruction
- foreign body
- large enteral defect >1 cm
- disruption of abdominal wall
- malnutrition
- sepsis
- high output fistula
