What do you use during skin assessment?
Wear gloves and use inspection and palpation
What are we checking for with skin?
Overall appearance- color, moisture, texture,
turgor, temperature- Normal-skin warm, dry and
intact
What are we checking for with hair?
note distribution, quantity, texture, and color
What are we checking for with nails?
Examine for color, shape, thickness, and contour
What can clubbed finger nails signify?
common in smokers and patients with
chronic respiratory distress orders (hypoxia).
Angle for clubbed nails?
Angle is greater than 18 degrees and nail is enlarged and curved
cyst
closed sac in or under the skin that contains

macule
small discolored spot or patch on the
skin
papule
a solid round, raised lesion that’s usually
less than 1 cm in diameter
wheal
raised, red area that’s commonly itchy and last for 24 hours or less

extends into the dermis
vesicle
small fluid filled blister that is usually 1 cm or less in diameter

ABCDE
what does ABCDE stand for?
asymmetrical lesions, irregular borders, color variations, diameter greater than 6 cm, elevated or enlarging lesions
