cardio assessment
Use inspection, palpation, percussion,
and auscultation.
Auscultate with the diaphragm to
hear
high pitched sounds
Auscultate with the bell to
hear
low pitched sounds
Auscultate the apical impulse at
the fifth intercostal space, left midclavicular line

- Normal finding in children and young adults
* Cardinal sign of heart failure

•Abnormal-heard over tricuspid or mitral area when
patient is lying on left side
•Heard in elderly or patient with hypertension, aortic
stenosis or history of myocardial infract (MI)
pericardial friction rub
Inflamed pericardial surfaces rub together and
produce a high-pitched friction noise that is grating
or scratchy. Best heard when patient is leaning
front.
Pulse 0-4
0+= absent 1+= weak 2+= normal 3= increased 4= bounding
Carotid pulses are palpated
separately
Risk reduction for coronary artery
disease (CAD)
- Eat low sodium foods
- Exercise at least 30 minute a day
- Manage stress
- No smoking
- No alcohol
- Low fat, low cholesterol diet
