ACL:
MOI, Sx, tests, treatment
MOI: Sudden deceleration, valgus force, twisting w/ planted foot
Sx: Swelling, instability, pain
Tests: Lachman’s, Anterior drawer, Sweep test
Treatment: Gentle PAMs, quad sets, general strengthining.
PCL
MOI, Sx, tests, treatment
MOI: Direct blow to tibia, hyperextension
Sx: Posterior knee pain, difficulty w/ knee flexion past 90
Tests: Posterior drawer, Dial test, rule out other ligaments
Treatment: Quad sets, avoid hamstring exercises, increase ROM, PAs and APs
Meniscus
MOI, Sx, tests, treatment
MOI: Twisting or pivoting injury
Sx: Locking, clicking, catching, joint tenderness
Tests: McMurray’s, Apley’s compression, Thessaly’s, joint line palpation
Treatment: PRICE, muscle sets, low impact activities, activity modification
PFPS
MOI, Sx, tests, treatment
MOI: Overuse, excessive loading
Sx: Anterior knee pain, stairs and prolonged sitting aggravate
Tests: Rule out other pathology - joint line palpation, meniscus tests, fxnl tests.
Treatment: Strengthen - partial squats, hamstring curls, Clamshells
Knee OA
MOI, Sx, tests, treatment
MOI: Degenerative, age, prior knee injury
Sx: Morning stiffness, crepitus, decreased ROM
Tests: Knee ROM w/ goni, distraction, joint line palpation
Treatment: Distraction, strength training, thermal modalities, weight discussion
Osgood-Schlatter disease
MOI, Sx, tests, treatment
MOI: Young athletes, repetitive traction at tibial tuberosity
Sx: Pain at tibial tuberosity, Swelling, tenderness
Tests: Palpate tibial tubercle, sweep test, fxnl tests.
Treatment: Quad stretch if tight, activity modification, Ice and/or NSAIDS
Hip OA
MOI, Sx, tests, treatment
MOI: Degenerative, age, previous hip injury
Sx: Morning stiffness, decreased ROM (IR and flexion especially)
Tests: Scour, resisted SLR, IR under 15, flexion under 115
Treatment: Distraction, knee extensions, hamstring curls, clamshells
Acetabular Labral Tear
MOI, Sx, tests, treatment
MOI: Trauma, repetitive microtrauma
Sx: Groin pain, clicking, locking
Tests: Scour, Fitzgerald’s, posterior impingement, resisted SLR, C-sign
Treatment: Activity modification (avoid deep flexion/IR movements), Hip and core - planks, glutes. NSAIDS, Surgery if conservative treatment fails
FAI
MOI, Sx, tests, treatment
MOI: Structural abnormality
Sx: Deep hip pain at end ranges
Tests: Scour, posterior impingement, Faber/FADIR, resisted SLR, C-Sign
Treatment: Clamshells, endurance work (inclined walking), activity modification, surgery.
Piriformis Syndrome
MOI, Sx, tests, treatment
MOI: Direct trauma, overuse or tightness
Sx: Buttock pain, radiating leg pain, prolonged sitting agg
Tests: FAIR, palpate piriformis, SLR/slump
Treatment: FAIR stretch, nerve glides, hip mobilization, take sitting breaks
Femoral Bone Stress Injury
MOI, Sx, tests, treatment
MOI: Overuse, endurance sports
Sx: Deep groin pain, worse w/ WB
Tests: Fulcrum, Patellar-Pubic percussion, ask about calorie intake
Treatment: If imaging confirms BSI, NWB. Refer to nutritionist, Pt education about gradual return to activity and how to increase load safely.
Muscle Strain
MOI, Sx, tests, treatment
MOI: Forceful contraction
Sx: Sudden pain at time of injury, tenderness and bruising, pain with active movements.
Tests: Resisted muscle testing, palpation
Treatment: PRICE, gentle ROM, isometric exercises
