Sjogren's Syndrome Flashcards
what is sjogren’s syndrome?
immune mediated destruction of exocrine glands
what part of the body is most commonly affected?
mucous membrane
keratoconjunctivitis sicca and xerostomia along with lymphocytic infiltration of the minor salivary glands, without another rheumatologic disease
primary sjogren’s syndrome
keratoconjunctivitis sicca and xerostomia along with a connective tissue disease
secondary sjogren’s syndrome
what are the 5 clinical features of sjorgren’s syndrome?
FAADD
Fatigue
Arthralgias
Arthritis
Dry mouth
Dry eyes
what are 6 extra-glandular manifestations of sjogren’s syndrome?
DR NIBS
Dysphagia
Raynaud’s phenomenon
Nosebleeds
Interstitial lung disease/nephritis
Brain fog/neuropathy
Salivary gland enlargement (parotid)
what can xerophthalmia/keratoconjunctivitis sicca lead to?
corneal ulcers/infections
what will the enlarged parotid gland show on microscope?
B-cell lymphoma
patient’s with an enlarged parotid gland have an increased risk of what?
non-Hodgkin’s lymphoma
what is the most common cause of death in patients that have an enlarged parotid gland?
malignancy
what will labs be in Sjogren’s syndrome? (4)
“RoLand”
+ ANA
+ RF
Anti-Ro antibody
Anti-La antibody
what will CBC show? (2)
normochromic anemia
leukopenia
test that evaluates tear secretions by the lacrimal glands
schirmer test
what 2 tissue can be used for a biopsy to confirm sjogren’s syndrome? what will it show?
lower lip
parotid
lymphocyte infiltration + gland fibrosis
what is the criteria needed to diagnose Sjogren’s syndrome?
EITHER:
(+) salivary gland bx
(+) anti-Ro or anti-La antibodies
+ ONE:
dry eyes > 3 mo
dry mouth wt swollen glands
(+) schirmer’s test <5mm
(+) decreased salivary flow <1.5 mL in 15 mins
