What is sjogren’s syndrome?
Chronic inflammatory disorder
What is the patho of Sjogren’s?
lymphocytic infiltration and destruction of exocrine glands (mainly lacrimal and salivary) with extensive fibrosis
What are different types of Sjogren’s?
primary or secondary (RA, SLE, SS)
What is the epid of sjogren’s?
middle aged females
What is the presentation of sjogren’s?
- Keratoconjunctivitis Sicca
- Xerostomia
- Parotid swelling
- Polyarthritis/arthralgia
- Raynaud’s phenomenon
- Vasculitis
- Peripheral neuropathy
What is Keratoconjunctivitis Sicca?
dry eyes
What is xerostomia?
dry mouth
What investigations are done for Sjogren’s?
- Schirmer’s test
- Autoantibodies
Other tests to consider: - Salivary gland biopsy
What does Schirmer’s test show in Sjogren’s?
if less than 5mm of paper wet in <5mins, positive test
What would salivary gland biopsy show?
Focal lymphocytic sialadenitis
What is the management for dry eyes?
- hypromellose (fake tears)
2. eye drops
What is the management for the dry mouth?
Salivary substitutes (sugar free gum)
What is the management of severe disease?
Immunosuppressants (steroids)
What are complications of Sjogren’s?
- Belpharitis
- Dental caries
- Blurring of vision
