ALL employees shall receive how many sick days per month ?

The Corp will STOP paying out sick time if (two things)
- If they provide proof of ?
- If they request how ?
If the employee is off sick for example an extended period of time
- The employee provides proof of reciept for LTD (long term disabilty)
- A request in writing to NOT recieve sick time pay
Corp wont pay sick time for duration of LTD

The crediting of, or the loss of accumulated sick leave credits shall NOT be adversely affected, when the employee is absent.
(you will not lose credits or stop accumlating if sick)

BFES is to make every reasonable effort to provide modified work. MODIFIED WORK is MANDITORY
- The CORP can require an employee to ACCEPT what ?
- But has to accomidate what as per human rights code?
- the Corporation can require an employee to ACCEPT suitable and available modified work,
- but also in the sense that, consistent with the duty to accommodate disability as defined by the Ontario Human Rights Code
**the employee is ENTITLED to the modified work**

- The objective is to return to full time duties by matching what? Identified by a health professional.
- So they will continue to accumulate what two things ?
- Permit a reasonable what ?
- Promotes an early return to suitable work, reduces cost, safeguards health, lessens adverse impact
- Return to full duties through matching functional ability and limitation as identified by health professional (objective of modified)
- They will continue to accure sick leave, seniority and service
- Permit a reasonable convalescence (time to get better)

- Diabled employee (don’t forget yourself)
- Health professional
- Department Representative and Fire Chief (or designate)
- Association Rep - President (or disignate)
- Health and Safety Rep
designate is probably the platoon cheif
**the collective says and fire chief, it also says and president (assuming two people each but it’s likely just one person)**

Responsibilties of the disabled
- Promptly report what ?
- Obtain what in order to recover ?
- Provide med information following what ?
- Maintain regular contact with whom ?
- Participate and explore what opportunities?
- Communicate difficulties to whom?

- Promptly report injuries and illness
- Obtain healthcare treatment and rehab to recover
- Provide med info with respect to return to work following initial assessment
- Maintain regular contact with department and WSIB
- Participate and explore modified work opportunities
- Communicate difficulties with TEAM
- Work with all members for a successful return

Department Rep (chief or delegate) responsibilties
- Maintain contact with the disabled employee to monitor his/her ability to return to work and for evaluation and support during the rehabilitation process
- Modify the work or workplace, as necessary and able, to accommodate the disabled employee’s functional abilities and limitations
- Provide training, as appropriate
- Monitor the disabled employee’s performance and progress in relation to his/her functional abilities or limitations
- Ensure that no tasks are being assigned other than those in accordance with the functional abilities and limitations on the Medical Assessment Form
- Relate progress evaluation and concerns regarding the modified work to the Health and Safety Services Representative and the Association Representative
- Work in cooperation with all members of the MWP Team to facilitate a successful return to work as soon as possible
- Advise the Association Representative whenever a disabled employee is placed on a modified work plan

The Health and Safety Services Rep responsibilites
- Facilitate what ?
- Receive and review what ?
- Coordinate meetings what kind of meetings?
- Act as a liason for what ?
- Work in cooperation with what team ?
- Facilitate the disabled employee’s early and safe return to work following an injury or illness
- Receive and review documentation regarding the disabled employee’s functional abilities and limitations, and progress
- Coordinate MWP Team meetings to discuss the disabled employee’s functional abilities and limitations, and employment alternatives
- Act as liaison between disabled employee, Association Representative, Department Representative, health professional, and WSIB, where applicable
- Work in cooperation with all members of the MWP Team to facilitate a successful return to work as soon as possible

The assignment of modified work to a PARTIALLY disabled employee does not create ?
