Shoulder Special tests Flashcards
Sulcas at 0
Positive: indentation, pseudo locking, pain
Indicates: laxity in in superior GH ligament
Sulcas at 90
Positive: indentation, locking, pain
Indicates: laxity in in inferior GH ligament
Anterior Load and Shift
Positive: pain, apprehension, painful click, excessive translation
Indicates: laxity in the anterior humeral ligaments/ anterior capsule
Posterior Load and Shift
Positive: pain, apprehension, painful click, excessive translaton
Indicates: laxity in the posterior humeral ligaments/posterior capsule
Compression/ Rotation
Positive: pain, clicking, pseudolocking
Indicates: SLAP lesion
Anterior/ posterior slide
Positive: anterior pain in GH extension or posterior pain in GH flexion
Indicates: SLAP lesion
Speeds
Positive: recreation of pain
Indicates: tendonitis of the biceps tendon
Obriens
Positve: pain to superior shoulder region while in IR resolved when ER
Indicates: superior shoulder= AC joint
Deep pain= SLAP
Yergasons
Positive: snapping in bicipital groove, pain
Indicates: tear or laxity in transverse humeral ligament or bicipital tendinitis
Anterior/Posterior Shear
Positive: pain in AC joint or excessive movement
Indicates: AC joint damage or damage to coracoclavicular ligaments
Neers
Positive: pain with motion (esp at end)
Indicates: impingement of supraspinatus or long hear of the biceps tendon
Hawkins and Kennedy
Positive: pain with motion
Indicates: impingement of the supraspinatus
Coracoid
Positive: anterior pain with motion
Indicates: subscapularis and long head of biceps tendon medially or supraspinatus impingement laterally
Cross-over
Positive: recreation of pain
Indicates: Anterior=subscap or long head of biceps
Lateral= supraspinatus
Superior= AC joint
Posterior= infraspinatus, teres minor or posterior capsule
Drop arm I
Positive: arm drops
Indicates: full thickness tear
Drop armII
Positive: arm drops at 90
Indicates: Full thickness rotator cuff tear
External Rotation Lag sign
Positive: angular drop greater than 5 degrees
Indicates: Part 1= infraspinatus tear
Part 2= supraspinatus tear
90/90 drop sign
Positive: drop occurs
Indicates: infraspinatus
Internal Rotation lag sign
Positive: lag occurs
Indicates: subscapularis tear
Apprehension sign
positive: apprehension
Indicates: anterior capsule and/or glenoid labrum may be compromised
Jobe sublaxation/relocation
Positive: anterior pain in first part followed by reduction of pain during relocation
Posterior pain with subluxation and reduces pain with relocation
Indicates: anterior shoulder microinstibility
OR internal impingement
Posterior glide at 90/jerk/ posterior glide at 0
Positive: pain, apprehension, clicking, instability
Indicates: posterior shoulder instibility
