Shoulder snd elbow problems Flashcards
name the muscles of the rotator cuff
supraspinatus and infraspinatus, subscapularis, teres minor.
what is seen in px of a rotator cuff tear?
muscular atrophy of the deltoid causing asymmarty, swelling
sy ellicited during examination of a rotator cuff tear?
pain and weakness on extesnsion and abduction and on internal and external rotation of the arm. Crepitus over the shoulder joint.
Special examination done?
Hawkin’s test, drop arm test for impingement.
which 2 other joints would it impact?
neck and elbow
shoulder arthritis…
most common is osteoarthritis, inflammatory arthritis, post-traumatic arthritis. Rotator cuff integrity is paramount. Tx - by 3d ct reconstruction
define golfers elbow
area of pain on the inside of thhe forearm
define tennis elbow
area of pain on the outside of the forearm
what is the percentages of traumatic dislocation on the shoulder ant, post, inf
ant - 90, post-9, inf- 1
what is the epidemiology of shoulder injuries?
Teens/20’s: Fractures and instability, 30’s & 40’s: Rotator cuff & capsulitis, 50’s & 60’s: Impingement and AC joint, 70’s + : Degenerative rotator cuff and joint
subacromial impingement…
Pain and dysfunction resulting from any pathology which decreases the volume of the subacromial space or increases the size of the contents. Painful arc. Tx - steroid injection, phhysio, arthroscopic decompression
frozen shoulder…
pain then stiffness then thawing, adhesive capsulitis, clinical diagnosis, early presentation shows hydrodilation, normal radiograph, later - surgery.
rotator cuff tear…
traumatic or degenerative. Acute tear requires surgery, chhronic degenerative tears not always unless symptomatic. Superior capsular reconstruction is a tx option (uses a caderveric skin graft to reconstruct the capsule).
