What is subacromial impingement ?

What often causes

A

Pain and dysfunction due to decreased volume of subacromial space or increased contents
e.g. swollen bursae or increased bone e.g. after injury
Often due to rotator cuff injury

A

Painful arc in abduction - 60-120 (rotator cuff tear = pain up to 60)
Initially pain free
Pain hanging washing up etc
May subside when end of arch reached

A

NSAID

A
Very acute shoulder pain 
Passive and active affected 
All movement affected
External rotation worse than internal / abduction 
Then becomes stiff
Gradually over 2 years ROM returns

What other tests to exclude

A

X-ray is normal
Exclude other causes - BG, TFT
Passive no better than active
Passive better if rotator cuff tear

A

Supraspinatous
Infraspinatous
Teres minor
Subscapularis

Stability of shoulder
Hold humerus against glenoid when deltoid abducts so may not be able to abduct even if deltoid fine

SIT = greater tuberosity of humerus 
Subscapularis = lesser tuberosity

A

Tendinopathy / degeneration of flexor muscles to medial epicondyle

A

Pain and tenderness over medial epicondyle
Worse on extension of elbow
Worse on wrist flexion / pronation
May have numb 4th + 5th fingers due to ulnar nerve

A

Tendinopathy / degeneration of extensor muscles to lateral epicondyle

Worse on flexion elbow (opposite to muscle inflamed)
Pain worse on resisted wrist extension
