Describing Fractures Flashcards

1
Q

What do you say at the start when describing X-ray

A
Views of X-ray e.g. AP
- Always require 2 perpendicular for ortho
- If give one in OSCE ask for another
Person name
Person age
Time
Part / anatomical site - make sure you know
Abnormality / fracture pattern
Example
AP + lateral X-ray of
Jane Doe 58 years
Taken yesterday
Shows a transverse fracture of tibia mid-shaft

What clinical features should you mention

A
Mechanism of fracture
- Trauma / stress / pathological
Energy
- High or low
Skin and soft tissue
- Open or closed
- Air suggest gas forming organism
- Fluid = haematoma
- FB

7
Q

What is articular involvement

A
Does fracture line extend into a joint
Intra-articular
- Dislocation
- Subluxation
Ligamentous
Epiphyseal

8
Q

What can shape of fracture or fracture pattern be

A

Transverse - perpendicular
Oblique
Spiral - severe oblique due to rotation
Multi-fragmented / Comminuted
Avulsion - tendon or ligament pulls bone
Impacted - bone squashed through fracture and in contact
Complex= mixed

15
Q

What is angulation

A
Angle between one segment relative to another typically distal segment
Medial or lateral and degrees
Dorsal vs volar if wrist
Colles = dorsal
Smith = volar (palm down)

16
Q

What is rotation

A

Internal - toward midline
External
Don’t usually comment

17
Q

What is an axial deformity

A

Any deformity along long axis of bone

Impaction = most common where two ends forced together

18
Q

What is displacement

A

Displacement + translation

  • Incomplete
  • Complete

Shift of one segment to another
Comment on direction and % relative to diameter of shaft
e.g anterior / posterior

19
Q

What is displacement

A

Displacement + translation

  • Incomplete

Shift of one segment to another
Comment on direction and % relative to diameter of shaft
e.g anterior / posterior

20
Q

What is rule of 2 for trauma

A

2 view - AP + lateral
2 joints - above and below
2 bones
2 occasions as may not show until later
