Define sexual orientation
Person’s attraction whether emotional, psychological or physical to people of same or opposite gender as identified by the individual
What is gender identity
A persons innate identification as a man or women which may or may not correspond to the sex assigned to them at birth
What is gender expression
External manifestations of gender, expressed through one’s name, pronouns, clothing, behaviour, body characteristics
What is a gender affirming surgery
Surgical procedure in which a person’s physical function and appearance of the existing sexual characteristics is changed to resemble that of the other sex
What is gender identity disorder or dysphoria
A desire to live and be accepted as a member of the opposite sex, usually accompanied by a sense of discomfort with one’s anatomical sex and wish to have surgery and hormonal treatment to make changes to be congruent with preferred sex
What is intersex
Person whose sex can not be classified as male of fe,able, may have both atttributes of both sexes or lack attributes which are considered necessary to define them as one or the other sex.
What are gatekeepers
Health workers who effectively block or limit trans people’s ability to obtain transition resources such as hormones or related services needed for physical gender affirmation transition
How can a facility or practice demonstrate an open and inclusive clinical environment for SGM patients
- All staff should be sensitised to demonstrate positive attitude and use sensitive language towards SGM pt
- Allow SGM to to self identify in new to forms if they choose to do so, include other on forms
- SGM rights should be included in facility’s anti discrimination policies and clearly displayed
- Posters and flyers reflecting SGM issues should be displayed in waiting room s
5, non judgemental attitude - Assure pt that consultation confidential
- Use gender neutral terminology eg partner nit wife
Why are MSM and gay more the most at risk of HIV and STIs
- Rectal tissue is friable and does not lubricate, can tear easily on penetration
- Increased substance use leading to risk taking behaviours and impaired judgement when intoxicated
- Some MSM do not think they are at risk since they don’t identify as gay
How can WSW practice safer sex
Wash hands and wear gloves before fingering
Use plenty of lubricant to prevent tears
Use condoms to cover sex toys, wash them
Use dental dam, condom cut open or cling film for oral sex
