Definitions.
Serious incident?
Involves death, serious injury hospitalization, officer involved shootings, pro long or violent tactical operations major disaster scenes etc.
Serious incident policy, why are these policies set in place?
The objective of this policy is to establish a procedure regarding departmental employees involved in serious incident and how and to whom notifications will be made.
Definitions.
Primary briefing?
Conducted by the on-scene supervisor at a confidential location i.e., inside a building, mobile activity command Mac van, etc. And will include a detailed dissemination of the information available on the Incident.

Secondary briefing?
Conducted by a person designated by the unseen supervisor and will be a general information session.
Notifications.
In addition to other notifications made by the duty commander and /or the communications bureau, the following notifications will be made by the duty commander or designee:
The Phoenix law enforcement Association on call representative will be notified for the list maintained by the duty commander designee for the communication bureau.

The phoenix police sergeant and Lieut. association PPSLA on call representative will be notified from the list maintained by the duty commander, designee, communications bureau.

One plea and/or PPSLA representative May respond to the scene and attend the secondary briefing conducted by a person designated by the on-scene supervisor.

The plea and/or PPSLA represent who responds to the scene will not be the representative who directly represents the affected employee during the investigation.
General procedures.
When employees are involved in a serious incident, a ? or designated ? will accompany the involved employee away from the scene to a quiet place i.e. a restaurant, etc. As soon as possible.

Gen. investigations bureau.
PSB bureau representatives.
Incident review unit representative.
Family members, if appropriate.
General procedures.
Employees will be ? and ? to contact their spouses or other family members as soon as practical after a serious incident.
Permitted and encourage
If the employee has been injured and feels that it will be useful, the employees family or friends will be contacted in person by a ? Who personally knows the involved employee and/or family.
Designated employee
The designated employee will provide necessary transportation and other requested assistance.
Employees will be permitted to contact a ? Or ? Representative if requested.
PLEA OR PPSLA
Prior to the investigation, conversation between the involved employee and clean and/or PPSL a representative will remain within the ? Of allowed representation.
