Septic Shock Flashcards
what causes toxic shock?
super antigens
gram +
stimulate lots of T cells
What is SIRS? What are the criteria for having it?
Systemic Inflammatory response syndrome tachypnea tachycardia fever 10% bands or wonky WBCs
What are the pro inflammatory cytokines?
IL 1, IL 6, TNF alpha
What causes septic shock when infected by gram - bacteria?
endotoxins
What are the most common gram -
E coli
Kleb
pseudomonas
What overall causes septic shock to be so sever?
over response of pro inflammatory cytokines
Where do gram - endotoxins bind? What is activated?
TLR and CD14; PRR and PAMPs
macrophages & NK cells
where do gram + super antigens bind? what is activated?
TLR & HLA II;
macrophages and CD4 +
not processed into superantigens
What is the purpose of IL 2?
stimulates other T cells
What bacteria is most implicated in toxic shock?
Staph. aureus
what type of hypersensitivity response is anaphylaxis? What is it mainly mediated by?
type I
IgE
Where does IgE bind to mast cells?
through the Fcepsilon receptor
What happens once IgE binds mast cells?
they degranulate
arachidonic acid is broken down
cytokines are released
vasodilation occurs
what is recognized by the body for septic shock?
PAMPs endotoxin LPS peptidoglycan DNA repeats
What is the pathophys of toxic shock?
superantigen creates crossbridge between CD4 T cell and APC
this activates them and leads to a cytokine storm
causes vasodilation and endothelial damage fluid leaks out
hypotension ensues
