What are the causes of septic arthritis?

Haematogenous direct trauma local infection iatrogenic adjacent osteomyelitis

What are the causative organisms?
A
Staph aureus is most common
Gonorrhoea in young adults

What criteria is used to diagnose SA? (and helps distinguish transient synovitis from SA in a child with an inflamed hip)
A
Kocher:
- Fever >38.5
- Non-weight bearing
- Raised WCC >12000 cells/mm3
- Raised ESR >40mm/hr

What are the clinical features of SA?
A
Pain referred to the knee limp red, hot tender joint reduced ROM Signs of infection Pseudoparesis

What are the investigations for SA?
A
Bloods: WBC, ESR, CRP, cultures
MSU
US of joint
Joint aspiration and culture

What is the management of SA?
A
- IV abx - fluclox or clindamycin
2. Joint washout and drainage if resolution isn’t rapid or joint is deep
