What is septic arthritis?
Infection within a joint
What are the routes of infection?
Haematogenous
Eruption of bone abscess
Direct invasion through penetrating wound
What are some causative organisms?
Staph aureus
Neisseria gonorrhoea
Group A strep
Haemophilus influenzae
E. coli
What is the pathology of septic arthritis?
Acute synovitis with purulent joint effusion
Complete destruction of articular cartilage
What is the presentation of septic arthritis?
Acute pain in a single large joint
Hot, red, swollen, painful
Stiffness and reduced ROM
Fever, lethargy, sepsis
What are some investigations?
FBC, WCC, ESR, CRP
Blood culture
X ray
Ultrasound
MRI
Aspiration and culture
What are differentials for septic arthritis?
Acute osteomyelitis
Trauma
RA
Haemophilia
gout
What is the management of septic arthritis?
Supportive measures
Empirical IV antibiotics for 3-6 weeks- flucloxacillin + rifampicin
