Presentation testicular torsion
Young boy
Sudden onset pain nausea, vomiting
High riding, swollen, tender testicle
Investigations with suspected torsion
Surgical exploration ASAP
Signs of appendix torsion
Burnt match head, blue dot sign
Clinical features epididymitis?
Gradual onset of pain
May have dysuria, urethral discharge
Tender swelling initially
Typically in sexually active men
What should we look for in testicular trauma?
Tunica rupture
Look for using ultrasound
Needs surgical repair
Fourniers gangrene
Polymicronbial scrotal infection
Edema, swollen, malodorous
Risks: diabetes, poor perfusion, elderly, recent procedure/intstrumentation
Causes of chronic scrotal pain
Granuloma Cystic lesion Hernia Epididymitis Idiopathic***
NO CURE!
Ddx painless scrotal mass
Hydrocele
Hernia
Spermatocele
Varicocele
Hydrocele
Fluid within tunica vaginalis
Transilluminates
Communicating Hydrocele
Incompletely closed processes vaginalis
Typically in kids
Scrotal swelling ,position dependant
Clinical presentation Varicocele
Common
Typically left sided
Generally asymptomatic
May be able to see “bag of worms” scrotum or may be palpable
