Scripts Flashcards
Normal MSK Xray
Osseous density is adequate. Gross osseous alignment is maintained.
No evidence of fracture, dislocation, or aggressive osseous pathology.
Joint spacing is preserved.
Soft tissue is free of abnormal calcification, ossification, or distention.
Normal Chest Xray
The lungs are well aerated and vascularized.
Costophrenic and cardiophrenic angles are sharp and clear of fluid.
There is no evidence of air space or interstitial disease.
There is no evidence of pulmonary mass or infiltrate.
The trachea is midline. There is no mediastinal widening.
The hila are normal in shape, size, and location.
The cardiac silhouette is of normal size and shape.
The visualized abdominal organs are unremarkable. No evidence of pneumoperitoneum.
The right and left hemidiaphragms are normal.
Osseous density is adequate.
No evidence of fracture, dislocation or aggressive osseous pathology.
Remaining extrapulmonary soft tissues are unremarkable.
Normal Ankle MR
No gross fracture, dislocation or aggressive osseous pathology is noted. Gross osseous alignment is maintained. Bone marrow signal is normal.
The medial and lateral ligamentous complexes of the ankle as well as the intrinsic ligaments of the hindfoot are intact.
The muscles and tendons of the ankle are normal in signal and morphology. The plantar fascia is normal.
The neurovascular structures of the ankle are unremarkable.
No excessive synovial or bursal fluid collections are noted.
Normal Brain MR
Cerebral hemispheres are symmetric. Gray-White differentiation
is maintained. No brain atrophy is present.
- Normal diffusion is seen (if applicable).
- Intracranial vasculature is unremarkable.
- The brainstem and cerebellum are intact and demonstrate normal
signal and morphology. Ventricular systems are intact.
- Vestibulocochlear system is maintained. Meckel’s cave is clear.
- Contents of the orbits are unremarkable. Paranasal sinuses are
well pneumatized and oral structures are maintained.
- The calvarium is normal in signal and morphology.
- The upper cervical spine and its cord are intact.
- Extracranial soft tissues are unremarkable.
Normal arthrogram
Examination is directed toward evaluation of intra-articular structures of the BLANK.
No extra-articular collections of gadolinum are identified.
No macroscopic cartilage defects are detected.
Normal Hip/Pelvis MR
Images are directed toward evaluation of the hip joints.
No fx or dislocations are detected.
There is minimal fluid in BLANK within physiologic limits without frank effusion.
Visualized portions of the muscles and soft tissues are within normal limits in signal intensity and morphology.
The visualized muscles appear symmetric
Examination of the pelvic structures are limited.
Normal Knee MR
No gross fractures, dislocations or aggressive osseous pathology is visualized. No subchondral marrow edema is identified. The patellofemoral and tibiofemoral cartilage is grossly preserved. No joint effusion is noted. No Baker’s cyst is present.
The quadriceps and patella tendons are intact and WNL in SI and morphology.
The medial and lateral menisci are WNL in SI and morphology.
The cruciate and collateral ligaments are intact and WNL in SI and morphology.
The posterolateral corner structures are intact.
Normal Cervical MR
Osseous alignment is maintained. There is no gross fracture, dislocations or aggressive osseous pathology. Bone marrow signal is WNL. The spinal cord is of normal SI and morphology. No caudal displacement of the brainstem or cerebellar tonsils are noted. There is no gross upper cervical or paravertebral pathology.
The remaining intervertebral disc heights and SI are WNL without evidence of disc bulge or protrusion. No central canal or neuroforaminal stenosis is appreciated.
Normal Lumbar MR
Osseous alignment is maintained. There is no gross fracture, dislocation or aggressive pathology. Vertebral body heights are maintained. Bone marrow signal is normal. The spinal cord terminates at L1 and is of normal signal intensity and morphology. There is no gross paravertebral pathology.
The remaining intervertebral disc heights and SI are WNL. No central canal or neuroforaminal stenosis is appreciated.
Normal Wrist MR
Osseous alignment is maintained. There is no gross fracture, dislocation or aggressive pathology. Hyaline cartilage is preserved
The TFC complex is intact. The interosseous ligaments are intact.
Carpal tunnel is WNL w/o evidence of convex bowing of the flexor retinaculum. The median nerve is of normal SI and morphology.
Visualized portions of the tendons crossing the wrist are WNL w/o evidence of tendon rupture, tendinosis or tenosynovitis.
No joint effusion or ganglion cyst is appreciated.
