Schizophrenia :
disorders are defined by 5 of the following domains:
lasts for how long:
1 or more but one must be 1-3 :
- Delusions, hallucinations, disorganized thinking, grossly disorganized or abnormal motor behavior and negative symptoms
- Symptoms persist for at least 6 months, 1 month active phase of symptoms
- level of functioning is affected
- schizoaffective disorder, depressive and biploar disorder with psychotic features have been ruled out
- not attributed to phsyciological affects or substances
- history of autism, the additional diagnosis of schizophrenia ???
Delusions
fixed beliefs that are not amenalbe to change in light of conflicting evidence.
Types of Delusions
Persecutory delusions (believes she is being conspired against) Grandiose delusions (Conviction of having great unrecognized talent) Erotomanic delusions (believes someone of high statue is in love with them) Jealous delusions (believes spouse is unfaithful) Somatic delusions (believes she experiencing bodily functions or sensations)
Hallucinations
perception-like experiences that occur without an external stimulus ; they are vivid and clear, with full force, impact normal perception and not under voluntary control; may be sensory modality but auditory hallucinations are most common; must occur while falling asleep or waking up
Disorganized Speech
Typically an individuals speech may switch from one topic to another; symptoms must be sever enough to impair communication
Grossly disorganized or Abnormal Motor Behavior
Childlike silliness to unpredictable agitation
Negative Symptoms
Diminished emotional expression: reductions in the expression of emotions in the face, eye contact, intonation of speech and movements of the hand, head and face that normally gave emotional emphasis
Avolition: a decrease in motivated self- initiated purposeful activities; may sit for long periods of time and show little interest in participating in work or social activities.
Others include: alogia (diminished speech output) anhedonia (decreased ability for pleasure) a-sociality (lack of interest in social interactions)
Delusional Disorder
- 1 month of the delusions and no other psychotic disorders
- schizophrenia criteria not met
- behavior is not obviously bizarre or odd
- Manic or major depressive episodes are brief
- disturbance is not attributed to drugs or medical conditions/cannot be better explained another mental disorder
*severity is rated by quantitive assessment of primary symptoms of psychosis
Brief psychotic Disorder
- sudden onset (within two weeks) of least 3 symptoms (delusions, hallucinations, disorganized speech, catatonic behavior )
- last more thank 1 day and remits by 1 month
- not better explained by depressive, bipoloar disorder with psychotic features or schizophrenia
Schizophreniform Disorder
- Two or more symptoms present ((delusions, hallucinations, disorganized speech, grossly disorganized, negative symptoms )
- lasts for 1 month period, but less than 6 months
- Schizoaffective and depressive and bipolar disorder have been ruled out
- not attribute to any physiological effects of a substance
Schizoaffective Disorder
- Uninterrupted period of illness during a major mood episode
- delusions or hallucinations for 2 or more weeks in the absence of a major mood episode during the lifetime duration of the illness
- symptoms meet critera for a major mood episode are present for the majority of the total duration of the active and residual portions of the illness
- not attributable to effects of a substance or medical condition
Substance/ Medication- Induced Psychotic Diorder
- Delusions and/or hallucinations
- no evidence of intxiatcation, symptoms couldn’t be produced by substance
- not better explained by psychotic disorder
- does not occur exclusively during the course of delirum
- causes signficant distress or impairment in social, occupational or other important areas of functioning
Psychotic Disorder due to other medical conditions
- Prominent hallucinations or delusions
- evidence from history that there is a direct pathophysiological consequence of another medical condition
- does not occur during course of delirum
- not better explained by another mental disorder
- causes significant distress or impairment in social occupational or other important areas of functioning
Cataonia with another mental disorder
