Pica
- Persistent eating nonfood substances
- over 1 month period
- not cultural
- no other mental disorder or medical condition (including pregnancy)
Specificy: in remission (if this is true)
Rumination Disorder
- repeated regurgitation of food
- over 1 month period
- not attributable to other medical condition
- eating disturbance does not occur during anorexia nervousa, bulima nervosa, binge eating disorder or avoidant/ restrictive
- symptoms occur in the context of another mental disorder
Specify :In remission
Avoidant/ restrictive Food Intake
A. Eating/ Feed disturbance (lack of interest in eating or food, avoidance based on sensory, failure to meet nutritional needs in one or more of the following:
- Significant weight loss
- Significant nutritional deficiency
- Dependence on enteral feeding or oral nutritional supplements
- Marked interference with psychosocial functioning
B. Disturbance not better explained by lack of available food or by an associated cultural sanctioned practice
C. No evidence of a disturbance in one’s weight or shape and does not occur during Anorexia or Bulima
D. Not attributable to concurrent medical condition
Specifcy: in remission
Anorexia Nervosa
A. Restricition of energy ( caloric) intake relative to requirements, leading to significantly low body weight for age, sex and physical health
B. intense fear of gaining weight
C. Disturbance in the way in which one’s body weight or shape is experienced undue influence of body weight or shape on self evaluation or persistant lack of recognition of seriousness of the current low body weight
Two Anorexia Types
Restricting Type: During last 3 months, has not engaged in recurrent binge eating OR purging behaviors; weight loss is accomplished through dieting, fasting and or excessive exercise
Binge- eating/ purging type : During last 3 months, the individual has engaged in recurrent episodes of binging, purging (induced) etc.
Anorexia Specifers
In partial remission: after diagnosis when weight is no longer deemed low or is sustained for a period of time but B & C are present.
In Full remission: none of criteria are met
Bulimia Nervosa
A. Recurrent episodes of binge eating
B. Recurrent compensatory behaviors
C. compensatory behaviors occur at least once a week for 3 months
D. self evaluation is influenced by body shape and weight
E. does not occur exclusively during episodes of A.N.
Difference between Anorexia Nervosa (binge-purging type) and Bulima?
B.N. tend to have normal weight where A.N. have significantly low
B.N. have compensatory behaviors at least once a week for three months, there is no time requirement.
B.N. is more prevenalant , has the highest death rates.
Etiology of Anorexia and Bulima
Sociocultural: admired people are thin, equating worth and beauty to limness, cultural affects: sports etc.
CBT: cognivitve bias reduces selfesteem / all or nothing reasoning, selective abstraction
Family System Theories: Eating disorders often surface in adolesence reflecting crises in particular development, families often place high emphasis on outward apperances
Treatment
Dialectical behavior: 4 domains to improve skills
core mindfulness, distress tolerance, emotion regulation, 4 interpersonal effectiveness
CBT Therapy
Binge Eating Disorder
A. Recurrent episodes of binge eating
B. Binge eating episodes associated with 3 or more
1. eating more rapidly than normal
2. eating until feeling full
3. eating large amounts of food when feeling physically hungry
4. eating alone because of embarrassment by how much one is eating
5. feeling disgusted with oneself, depressed or very guilty afteward
C. Marked distress regaurding binge eating is present
D. Binge eating disorder occurs, at least once a week for 3 months
