Disruptive Mood Dysregulation Disorder
Severe recurrent temper outbursts (verbal rages, or physical aggression that are out of proportion in intensity or duration to the situation or provocation
- outbursts inconsistant with development level
- outbursts occur on average 3 or more times per week
- the mood between temper outburst is persistently irritable or angry most of the day nearly every day and is observable by others
- symptoms present for more than 12 or more months, has not had a period lasting 3 more consecutive months without all symptoms listed above
- symptoms present in at least two of three settings (home, school, peers) and one is severe
- diagnosis should not be made before age 6 or after 18
- onset age of symptoms begins at 10 years
- never been a period lasting more than 1 day during which the symptoms could meet criteria of manic or hypomanic episode, or occur during an episode of major depressive disorder, OR not better explained by another mental disorder or attribute to a substance or neurological condition
Differential Diagnosis of Disruptive Mood Dysreuglation Disorder: Oppositional Defiant Disorder
differences are
- between the the prescence of sever and frequently recurrent outbursts and persistant disruption in mood between outbursts.
- ddmd requires one severe impairment
Differential Diagnosis of disruptive mood dysregulation disorder: ADD, ADHD, major depressive disorder, anxiety disorder and autisim spectrum
- children whose irritability is only present in the content of a major depressive disorder should receive those diagnosis rather than DMDD
Differential Diagnosis of Disruptive mood dysregulation disorder: Intermittent expolosive Disorder
- unlike disruptive mood dysregulation disorder, intermitten expolisve disorder does not require presistent disruption in mood between outbursts; also only requires 3 mornths of active symptoms where in contrast disruptive mood disorder criteria calls for 12 months active.
Major Depressive Disorder
Five or more of the following symtpoms have been present during the same 2-week period
- at least one of the symptoms is 1. depressed mood OR 2. loss of interest or pleasure
- depressed mood most of the day
- markedly diminished interst or pleasure in all activities
- signficant weight loss when not dieting or weight gain OR decrease or increase in appetite
- insomonia or hypersomnia
- Psychomotor agitation or retardation nearly every day.
- Fatigue or loss of energy nearly every day
- Feeligns of worthlessness or excessive or inapporiate guilt
- Diminshed ability to think or concentrate or indecivenss
- recurrent thoughts of death
- symptoms must cause significant distress or impairment in functioning
- not attributional to any other medical condition
- occurrence of major depressive episode is not better explained by other disorders and has never been a manic or hypomanic episode
Persistent Depressive Disorder (Dysthima
- depressed mood for most of the day for 2 consecutive years
- two or more of the following
1. Poor appetite or overeating
2. Insomonia or hypersomnia
3. Low energy or fatigue
4. self esteem
5. poor concentration or difficult making decisions
6. feelings of hopelessness
-during the 2 year period of the disturbances the individual has never been without the symptoms in criteria A & B for more than 2 months at time
- Criteria for a major depressive order may be continuously present for 2 years
There has never been a manic episode or hypomanic
- The disturbances is not better explained by a other disorders, substances
- symptoms cause clinacally signaficant distress or impairment in functioning
Premenstrual Dysphoric Disorder
At least 5 symptoms present before the onset menses, start improve within a few days after and become minimal or absent in the week post:
one ore more must present:
- marked affective lability
- marked iritabililty or anger
- marked depression mood
- marked anxiety
one or more of the following symptoms when combined with symptoms above
- decrated interest in activities
- subjective difficulty in conentration
- lethargy, easy fatigability, or marked lack of energy
- marked change in appetite; overeating or cravings
- Hypersomnia or insomnia
- Feelings of being overwhelmed or out of control
- Physical symptoms: tender breasts, joint pain, bloating, weight gain
- Symptoms are associated with clinically significant distress in function
- not merely an exacerbation of the symptoms of another disorder
- at least 2 symptomatic cycles
Recurrent Brief Depression
depressed mood at least 4 symptoms for 2-13 days at least at least twice per month for 12 consecutive months
short-duration depressive episode
Depressed with 4 of the 8 major depressive episodes with clinically significant distress or impairment that persits for more than 4 days but less than 14
depressive episodes with insufficient symptoms
meets 1 or 8 criteria of major depressive episode with distress or impairment in functioning
