How is schizophrenia defined?
By abnormalities in 1 or more of the 5 domains:
- Delusions
- Hallucinations
- Disorganized thinking (speech)
- Disorganized/abnormal motor behavior
- Negative sx
What is the DSM5 criteria for delusional disorder?
Presence of 1 (or more) delusions w/ a duration of 1 month or longer
What are the 7 subtypes of delusional disorder?
- Erotomanic
- Grandiose
- Jealous
- Persecutory
- Somatic
- Mixed
- Unspecified
What is the tx for delusional disorder?
- Challenging to tx
- Establish an alliance & negotiate tx goals
- Med for sx of depressed mood or irritability
- Outpt (unless harm to self or others)
- Tx co-morbid psychiatric disorder
- Antipsychotics
- Psychotherapy
What is the DSM5 criteria for brief psychotic disorder?
Presence of 1 (or more) of the following sx:
- Delusions
- Hallucinations
- Disorganized speech
- Disorganized or catatonic behavior
* At least 1 must be (1), (2), or (3)

What is the DSM5 criteria for schizoaffective disorder?
An uninterrupted period of illness during which there is a major mood episode (major depressive or manic) concurrent w/ Criterion A of schizophrenia.

Define: persecutory delusions
Belief that one is going to be harmed, harassed by an individual or organization
- Most common type
Define: erotomanic delusions
False belief that another person is in love w/ him/her
Define: grandiose delusions
Belief that one’s self has exceptional abilities, wealth, or fame
Define: nihilistic delusions
Conviction that a major catastrophe will occur
Define: somatic delusions
Preoccupations regarding health & organ fxn

Belief that one’s spouse/lover is unfaithful

When no one delusional theme predominates

When are delusions deemed bizarre?
If they are implausible, not understandable, & not derived from ordinary life experiences
Define: perceptual distortions
- Being distressed by hallucinatory voices, hearing voices that accuse, blame, or threaten punishment
- Seeing visions
How do you tx brief psychotic disorder?
- Prevent pts from harming themselves
- Consider hospitalization
- If pt is aggressive –> seclusion or restraint
- Remove stressor
- Antipsychotics (no longer than 1 month)
How do you tx schizoaffective disorder?
- Pharmacotherapy & psychotherapy
- Neuro consult to r/o neurologic disease
- Inpatient admission for pts suicidal, homicidal, or disabled
- Smoking cessation
- In depressive subtype: Combo of antidepressants (SSRIs) & antipsychotics
Screening tools for schizoaffective disorder?
- Positive & negative sx scale (schizo)
- Hamilton depression & Young mania scales (depression & bipolar)
- The CAGE Questionnaire (alcohol consumption)
Define: schizoaffective disorder
Mental illness that has both features of schizo (hallucinations, delusions, & distorted thinking) & features of mood disorder (depression or mania)
What is required for dx of schizoaffective disorder?
Pt must have psychosis for at least 2 wks w/out a mood disorder
What are the subtypes of schizoaffective disorder?
- Bipolar
- Depressive
