During last 3 mos, pt has engaged in binge eating or purging.
How do you specify current severity for anorexia nervosa w/ regards to BMI?
- Mild = > 17
- Moderate = 16-16.99
- Severe = 15-15.99
- Extreme = < 15
Describe “partial remission” w/ regards to anorexia nervosa.
Pt does not have low body weight, but still fears weight gain or still has disturbed perception regarding her weight/shape.
- Therefore, criteria A is not met, but criteria B or C is met.
Describe “full remission” w/ regards to anorexia nervosa.
None of the criteria are met for a sustained period of time
What are screening tools for eating disorder co-morbidities?
- Depression (PHQ9)
- Anxiety (GAD-7)
- OCD (Yale-Brown Obsessive Compulsive Scale)
- PTSD (7-item Breslau screen)
Sx of avoidant/restrictive food intake disorder
- Weight loss
- Nutritional deficiency
- Dependence on enteral feeding or oral supplements
Bulimia nervosa: What is the female athlete triad?
Eating d/o + hypothalamic amenorrhea + osteoporosis
Other signs/sx of bulimia nervosa
- Food gone in short time
- Frequent bathroom trips
- Laxatives, diuretics
- Jaw/cheek pain or swelling
- Russell’s sign
- Stained teeth
- Withdrawal from friends
What is the workup for eating disorders?
- CBC
- CMP
- Lipid
- Prealbumin, Mg, Phosphate
- TSH
- Hormones
- EKG
What does anorexia nervosa look like on PE?
- Emaciation
- Edema
- Decreased estrogen, osteoporosis, decreased muscle mass
- Dehydration
- Yellow skin
- Hirsutism, hair loss, brittle nails
- Hypotension, brady
- Amenorrhea
