Types of schistosomiasis and geography
Schistosoma Hameatobium: found in Africa and Middle East
Schistosoma Mansoni: found in africa and South america
Schistosoma Japonicum: found in China and Philippines
S Mekongi: similar to s japonicum, found in Laos
Schistosoma Haematobium/S Bovis: Corsica
Diagnostic stage and infective stage of Schisto
Dx- ova in stools (serology in expats)
Ix- cercariae
Actually Schisto presentation?
Known as Katyama fever:
Fever, fatigue, hepatosplenpmegaly and EOSINIOPHILIA
S. Haematobium presentation:
Actue: haematuria, UTIs, dysuria
Chronic: SCC bladder, hydroureter and chronic salpingitis
S Mansoni presentation:
Acute: abdo pain, colonic polyps
Chronic: portal fibrosis and portal hypertension, hepatosplenomegaly and variceal bleeding
S Japonicum presentation:
Acute: colonic polyps and abdo pain
Chronic: can be neuroschistosomiasis, typically eggs in brain, also get portal fibrosis and hypertension
How do expats normally present?
Usually asymptomatic with positive serology. tx praziquantel
Tx of Schisto:
Praziquantel, wait 6-8 weeks following exposure as only kills adults
Prednisone for katyama fever
