The referral cannot be from out of state, with the exception of physicians from neighboring states or have proof of residency in neighboring states.

It is not required; however, it is common and in some institutions required to do so.

It is not required; however, for reimbursements through insurance, it is required. Hence, it is commonly done.

False. Only PTs can employ and supervise a PTA.
