Symptoms
A
- fever
- malaise
- cough
- weight loss
- dyspnea
- arthritis in ankle and legs
- 50% of cases are incidentally on CXR

Physical findings
A
Can be GRUELING:
- Granulomas
- Rheumatoid arthritis
- Uveitis
- Erythema nodosum
- Lymphadenopathy
- Interstitial fibrosis
- Negative TB
- Gammaglobulinemia

CXR findigns
A
- Stage 1: bilateral hilar lyphadenopathy
Stage II: hilar adenopathy and pulm infiltrates - Stage III: isolated pulm infiltrates

Lab findings
A
- Serum ACE elevated
- hypercalcemia, hypercalciuria
- increased alk phos
- lymphopenia
- may show leukopenia
- Elevated ESR
- Skin test anergy
- serum protein electrophoresis will show polyclonal gammopathy

PFT findings
A
- Restrictive pattern
- FEV/FVC increases to 100%
- Decreased TLC
- decreased diffusion capacity

Bronchoalveolar lavage findings
A
increased lymphocytes and high CD4/CD8 ratio

Treatment
A
Systemic corticosteroids
good prognosis.
