- Granulomatous disorder - accumulation of granulomas and T lymphocytes

What is the aetiology of sarcoidosis?

A
  • Unknown cause
  • 75% age between 20-40yrs
  • Can affect all sexes, ages, races and places of origin
  • Black Africans highest rates

What are the symptoms of sarcoidosis?

A

DEPENDS ON ORGAN INVOLVEMENT

Respiratory =

  • Dry cough
  • Chest pain
  • Dyspnoea (usually progressive)
  • Fatigue
  • Malaise
  • Weight loss

What are the sign of sarcoidosis?

A
  • Lymphadenopathy
  • Crackles on auscultation
  • Weakness
  • Erythema nodusom + bilateral hilar lymphadenopathy = pathogonomic

What are the investigations for sarcoidosis?

A

Blood

  • Ca2+ = increased
  • Serum ACE = increased (poorly specific but good disease monitoring)

Imaging

  • CXR (bilateral lymphadenopathy, infiltrates, fibrosis)
  • Echocardiogram

What is the management for sarcoidosis?

A

Majority do not need treatment

Persistent symptoms = steroids then slowly tapered down

Resistant symptoms = methotrexate/chloroquine
