inflammatory condition - oresence of non-caseating granulomas (small patches of red,swollen tissue)
multi-systemic disease
cause?
type 4 hypersensitivity reaction - T-cell mediated
typical patient?
young, female with family history
features of acute sarcoidosis?
swinging fever
polyarthralgia
erythema nodosum
features of insidious sarcoidosis?
non-productive cough
SOB
fatigue
other features?
uveitis
cervical and submandibular lymphadenopathy
lupus pernio (lupus type rash)
diagnosis?
largely clinical
no single diagnostic test-
bloods, serum ca, ACE, CXR
CXR - used for staging - 0 to IV
management?
- observation
- steroids - inhaled budesonide, oral prednisolone
- immunosupression - methotrexate, azathioprine
complications?
resp - pulmonary HTN, resp failure, fibrosis
CV- cor pulmonale, heart block
CNS- cranial nerve palsies
ocular- uveitis, keratoconjunctivitis sicca
