what is it?
A
increased resistance and pressure of blood in the pulmonary arteries which causes strain on the R side of the heart
also causes a back pressure of blood into the systemic venous system

usually split into 5 groups. primary pulmonary HTN lupus left heart failure COPD PE sarcoidosis glycogen storage disease haematological disorders

A
SOB tachycardia peripheral oedema raised JVP hepatomegaly

CXR changes?
A
dilated pulmonary arteries
right ventricular hypertrophy

A
right ventricular hypertrophy
right axis deviation
RBBB

management?
A
IV prostanoids (e.g. epoprostenol)
Endothelin receptor antagonists (e.g. macitentan)
Phosphodiesterase-5 inhibitors (e.g. sildenafil)
treat underlying causes
montior for complications
