Salivary Gland Flashcards
What are the major salivary glands?
submandibular, parotid, sublingual
What are the minor salivary glands?
upper aerodigestive tract, lips mucosa oral cavity, pharynx and hard palate
What is the submanidbular duct?
warton
What is the parotid duct?
stenson
What history do you want for suspected salivary gland disease?
vaccines, radiation, PMH, overlying skin changes, foul taste, discharge, swelling or pain
What exam do you need to look for on salivary gland disease?
look for infection and stone
palpate glands and ductal opening
milk gland to see clear drainage
palpate floor and cheek
quick CN exam - CN 7 goes through parotid
look at eyes for interstitial keratitis - sjogrens
How do you dx salivary disease?
culture secretions, get CBC/electrolytes, blood cultures, ANA, SS-A/B, ESR
Fine needle biopsy if mass or abscess suspected
What is acute inflammation of salivary glands?
sialadenitis
What glands are usually involved in sialadenitis?
parotid and submandibular
What can cause sialadenitis?
dehydration, chronic illness, sjogren syndrome, ductal obstruction, s. aureus
What is the treatment for sialadenitis?
Clindamycin (900 mg IV q8h or 300 mg PO q8h) x10 days
Hydration; warm compresses; sialogogues;
What is a complication with sialadenitis?
abscess formation
What is the treatment for acute sialadenitis?
Hydration, antibiotics (oral versus parenteral), warm compresses and massage, sialogogues
Clindamycin (900 mg IV q8h or 300 mg PO q8h) 7-10 days
What is the surgical management of sialadenitis?
incision and drainage vs excision of gland in refractory to ABX
ID with abscess formation
What is recurrent enlargment of the gland typically without erythems?
chronic sialadenitis
