WHAT IS A CLASS “B” MISHAP?
TOTAL COST OF MATERIAL OR PROPERTY DAMAGE IS $500K OR MORE, BUT LESS THAN $2 MILLION; OR AN INJURY OR OCCUPATIONAL ILLNESS RESULTS IN PERMANENT PARTIAL DISABILITY , OR THREE OR MORE PERSONNEL ARE INPATIENT HOSPITALIZED.
