Define a Safe Culture:
Helping patients when they come to us and avoiding injury

Adverse event:

Sentinel event:
Unexpected occurrence involving death or serious, physical, or psychological injury, or putting someone at risk for it.
Very serious- that’s what differentiates it from an adverse event
