Sacroiliac Joint Dysfunction Flashcards
1
Q
What is the umbrella term for painful SI joint problems?
A
Sacroiliac Joint Dysfunction
2
Q
What is too much movement?
A
Hypermobility
3
Q
What is to little movement?
A
Hypomobility
4
Q
What is the etiology of HYPERmobility?
A
- Trauma
- Pregnancy
- Hypomobility in surround joints
- Increased flexibility due to body type of occupation
- Pathology
5
Q
What is the etiology of HYPOmobility?
A
- Soft tissue contractures
- Hypermobility in surround joints
- Decreased flexibility due to body type or occupation
- Surgical fixation
- Trauma, surgery and immobilization resulting in articular adhesions
- Pathology
- AgingW
6
Q
What are signs & symptoms of hypermobility?
A
- Tenderness @ sacral sulcus
- Grater than normal ROM
- May be painful if the ligaments are overstretched
- Will be painless if the ligaments are ruptured
- Hypertonicity of muscles crossing the joint
7
Q
What are signs & symptoms of HYPOmobility?
A
- Decreased ROM
- Stiffness & pain
- Adhesions & fibrosing
- Fascial restriction
- Myofascial pain including TPs in muscles crossing the joint
8
Q
What are CI’s & precautions for hypermobility?
A
- Do not mobilize
- Do not stretch
- Do not completely release mm’s that cross
9
Q
What are CI’s & precautions for HYPOmobility?
A
- Do not mobilize the joint that has been surgically repaired
- Do not restore full ROM in ligaments that have been surgically shortened
10
Q
What are assessment findings in HYPERmobility?
A
- Abnormalities
- ROM greater than normal
- palpate SIJ when performing hip/lumbar - Palpation : tender @ sacral sulcus, ropey, cool d/t ischemia, HT, TrPs in mm crossing the joint
- Strength Test : Weak muscles crossing the joint
- Length Test : Muscles cross the joint may be long
- Special Test :
- aSLR
- Thigh thrust
11
Q
What are assessment findings in HYPOmobility?
A
- Abnormalities may be present
- Limited ROM
- Palpate SIJ when performing lumbar & hip - Palpation : tender @ sacral sulcus, HT, TrPs, fascial restriction in mm crossing the joint
Strength Test : mm crossing the joint weak
Length : mm crossing the joint short
Special Test :
- Gillets’s
12
Q
What are massage goals in HYPERmobility?
A
- Decrease pain : diaphragmatic breathing
- Reduce HT & TPs
- Stimulate & strengthen long, weak, muscles : tapotement, vibrations, cold pack, kneading, stripping, positional release
- Normalize rom
Abdominal bolster
13
Q
What are massage goals in HYPOmobility?
A
- Decrease pain
- Reduce HT & TPs
- Decrease adh & fascial restrictions
- stripping, kneading, o+i, met/pnf, stretching - Lengthen short muscles
- Normalize ROM & joint mobility
Abdominal bolster
14
Q
What are homecare exercises for HYPERmobile joints?
A
- Strengthening muscles that cross the joint
- core, hip stabilizers
- hip flexor strength, glute bridge, deadbugs, bird dogs - Cold to decrease pain or stimulate
15
Q
What are homecare exercises for HYPOmobile joints?
A
- Self mobilizations
- Foam rolling
- Stretching
- Erectors, adductors, hamstrings, quads, glutes
- supine knee to chest
- seated/standing forward fold - Heat to decrease tone & muscle pain
- Contrast to increase circulation
- Progress to strengthening
