What defensive mechanism does gastric mucosa have from acid and H. Pylori?
Constant cell replication
Mucus membrane barrier
Good vascular supply
Describe the features of PPIs
E.g omeprazole, lansoprazole. Oral
Inhibits proton pumps reducing acid secretion
Describe the features of H2 receptor antagonists
E.g. ranitidine, Oral
Histamine can’t activate parietal cells to secrete acid. Less effective as only indirectly block proton pumps
What is H. Pylori?
Gram negative bacteria found in GI tract, which can cause gastric and duodenal inflammation/ulceration.
Toxic to epithelial cells and degrades mucosal lining
How do we diagnose the presence of H. Pylori?
Carbon-13 urea breath test and Stool antigen test.
What is the treatment for H. Pylori?
Clarithromcin
Amoxicillin
Lansoprazole (or metronidazole)
What is the treatment for GORD?
Lifestyle: weight loss, alcohol reduction.
Then H2RA, then PPI
What is the treatment for peptic ulcers?
Give H2RA and PPI for 6 weeks, to eradicate H. Pylori
