What is rheumatoid arthritis?
Autoimmune condition causing chronic inflammation of the synovial lining of the joint, tendon sheaths and bursa

Presentation of RA
Symmetrical, polyarthritis of the small joints of the hands and feet Joint pain (worse at rest or inactivity) Joint swelling (boggy) Joint stiffness (>30mins in morning)
What joints are affected in RA?
Wrist, ankle, MCPs, PIPs, knees, shoulders, elbows
What joint deformities can be seen in RA?
Ulnar deviation
Swan neck deformity
Boutonniere deformity
Z-shaped thumb
What is swan neck deformity?
Hyperextension at PIP with flexion at DIP
What is boutonniere deformity?
Flexion at PIP with hyperextension at DIP

1st line treatment for RA
Monotherapy Methotrexate Leflunomide Sulfasalazine Hydroxychloroquine
2nd line treatment for RA
Any 2 of Methotrexate Leflunomide Sulfasalazine Hydroxychloroquine Used in combination
3rd line treatment for RA
Tried combination therapy of 2 DMARDS and still has a DAS28 score >5.1
Methotrexate + biologic (usually TNF-inhibitor)
4th line treatment for RA
Methotrexate + rituximab
