which breakdown product is most dangerous to the kidney?

A

Myoglobin is toxic to the kidneys in high concentrations and causes AKI, which then causes the products to accumulate even more in the blood

Aetiology?

A
  • Prolonged immobility
    • esp important in frail/elderly who fall
  • Strenuous exercise beyond person’s fitness level
  • Crush injuries
  • Seizures

Signs and Symptoms?

A
  • muscle ache & pain
  • oedema
  • fatigue
  • red-brown urine

Ix?

A
  • CK
    • this will be in thousands/hundreds of thousands
  • Urine dipstick- Myoglobinuria (red brown urine)
    • will test positive for blood on dipstick
  • U&E
    • kidney function & K levels
  • ECG- check for heart’s response to hyperkalaemia
