what is it?
A
skeletal muscle breaks down and breakdown products released into blood

what are the muscle break down products?
A
- CK
- Myoglobin
- phosphate
- potassium

Q
which breakdown product is most dangerous to the kidney?
A
Myoglobin is toxic to the kidneys in high concentrations and causes AKI, which then causes the products to accumulate even more in the blood

Aetiology?
A
- Prolonged immobility
- esp important in frail/elderly who fall
- Strenuous exercise beyond person’s fitness level
- Crush injuries
- Seizures

Signs and Symptoms?
A
- muscle ache & pain
- oedema
- fatigue
- red-brown urine

Ix?
A
- CK
- this will be in thousands/hundreds of thousands
- Urine dipstick- Myoglobinuria (red brown urine)
- will test positive for blood on dipstick
- U&E
- kidney function & K levels
- ECG- check for heart’s response to hyperkalaemia

Mx?
A
mainstay- IV fluids
consider…
- IV sodium bicarbonate
- IV mannitol
