what are the advantages of the inhalation route?
Delivers drug directly to the airways
- smaller doses required than by mouth.
- less side effects
e.g. salubutamol more effective in inhaler from than the tablet form known as salvent

what is the issue with PMDI
elderly and children struggle to coordinate
Examples of PMDI?
salbutamol, seritide
if it says beclometasone inhaler on RX, do we give Clenil or QVAR?
you would give clenil, as QVAR is more potent and wouldnt match the dose of beclometasone.
QVAR is twice as potent than clenil, so they can’t be interchanged
what is the advantage of spacers?
remove the need for accutation and coordination
- allows larger proportion of particles to be inhaled and deposited in the lungs
- the larger the spacer the more effective is. Volumetric is most effective
what are the age restrictions on clenil and QVAR?
Clenil can be used in under 12’s
QVAR cant !

DPI’s are useful in which patients?
- adults and children 5 plus years
- unable to coordinate PMI i.e arthritis, too weak to press
How to take PMI?
- Stand or sit up straight
- breathe out until you cant breathe out anymore
- then use the inhaler breathe in for at least 10 seconds
how do you use symbicort dpi
take lid off
twist the bottom
this gets the dose ready
then breathe in sharp and hard
How do you use breath actuated?
Just pull lid off and breathe straight in
How can you reduce candidiasis when using ICS?
Rinse mouth with water after inhalation
- use a spacer
- anti-fungal oral suspension or GEL can be used to treat oral candidiasis without stopping the steroid
