Trichomoniasis
STI (flagellated, anaerobic protozoan) Vaginal discharge that is profuse, frothy, green in color, may have odor. PH may be less than 5. Treat with metronidazole 2g once PO or 500mg PO BID x 7 days.
Gonorrhea
STI, caused by Neisseria gonorrhoeae, common co-infection with chlamydia so always treat for both.
Chlamydia
Most common STI, caused by chlamydia trachoma’s, often accompanies gonorrhea,, tx with azithromycin 1mg PO once.
Ectopic pregnancy
pelvic and abd pain and unexplained vaginal bleeding the primary symptoms. A ruptured ectopic pregnancy is characterized by sudden onset of vaginal bleeding, sharp, severe, unilateral abd pain. Following rupture you can have sig blood loss causing shock resulting in hypotension, shoulder pain, breast tenderness. Physical findings suggestive of ectopic pregnancy include: CMT, uterus that is not enlarged, adnexal mass, adnexal tenderness. Dx with transvaginal US, pregnancy test, CBC, beta hcg. Tx refer to get methotrexate to dissolve pregnancy. MEDICAL EMERGENCY
