Gestational DM screening

A

Done between 24-28 weeks. Give 50gm oral glucose and do venus blood glucose 1 hour after, if BS greater than 130-140 then do 3 hour glucose test and give 100gm oral glucose. The dx of GDM is made with two elevated blood glucose values.

Preeclampsia

A

systolic of 140 or higher and a diastolic of 90 or higher, proteinuria of 1+ on 2 occasions each 4 hours apart or 300mg or greater in a 24 hour time frame or absent proteinuria but the presence of thrombocytopenia, renal insufficiency, impaired liver function, pulmonary edema, CNS or vision changes.

HEELP syndrome

A

Serious complication of preeclampsia, can still occur without preeclampsia, characterized by HTN, elevated liver enzymes, low platelets. May complain of HA, epigastric or chest pain, extreme fatigue, nausea, vomiting and is associated with hepatic rupture, DIC, and eclampsia

Placenta previa

A

painless vaginal bleeding is hallmark for placenta previa. bleeding may occur after 24 weeks and is usually bright red. Cervical exams are contraindicated with active vaginal bleeding.

Abruptio Placentae

A

Associated with painful, bright red vaginal bleeding and sudden onset of sharp, localized abd pain. Mild back pain or abd cramping with no vaginal bleeding may also be associated with abrupt placentae. The abd will be board-like firm and FHR may be diminished or absent.

Placenta Accreta

A

villi adhere to myometrium, placenta increta extends into the myometrium, placenta accrete extends through the myometrium. the more C sections a woman has the greater the risk of developing this. Vaginal bleeding can be a sign of this.
