WHAT TIME IS CREW MEMBER EXPECTED TO REPORT FOR DUTY?
FOM 04.14 OF 21 CREW MEMBERS ARE EXPECTED TO REPORT FOR DUTY AT THE SPECIFIED TIME INDICATED ON THE CREW MEMBERS SCHEDLULE. THIS TIME AMREKS THE BEGINNING OF A CREW MEMBERS DUTY DAY FOR THAT DUTY PERIOD. Typically 45 minutes prior to departure, and inside security. 1 Hour international.

WHAT ACTION SHOULD BE TAKEN TO MODIFY A REPORT TIME DUE TO LATE ARRIVAL AT AN OVERNIGHT STATION?
WHEN REPORT TIME IS MODIFIED DUE TO LATE ARRIVAL AT THE OVERNIGHT STATION. THE PIC COORDINATES WITH CREW SUPPORT TO DETERMINE A REPORT TIME CONSISTENT WITH THE REQUIRED REEST PERIOD.
- FO – When is a crewmember released from duty?
FOM 4.15 21 Release Time
1) Crewmembers are released from duty 15 minutes after the completion of the last scheduled flight or aircraft movement.
How is the fit for duty affirmation accomplished?
FOM 4.4 21 Prior to commencing a flight segment, each flight crewmember must affirmatively state they are fit for duty, pursuant to the definition.
(1) Fit for duty affirmation is accomplished by:
(a) EFB connectivity – Activating for a flight segment in the FliteView application.
(b) No EFB connectivity – Contacting the dispatcher for a manual activation; both crew members must independently state each is fit for duty.
Upon activation, each crew members’s electronic signature affirming fit for duty is permanently attached to the release signatory document. Additionally each dispatch release is affixed with a fit for duty affirmation statement.
FO – In the absence of EFB connectivity, what must be done to accomplish the fit for duty affirmation?
No EFB connectivity – Contacting the dispatcher for a manual activation; both crewmembers must independently state each is fit for duty.
CA – What documents are crewmembers required to possess when reporting for duty for a domestic flight?

Each crewmember ensures they possess the following required items prior to departure:
1) Current pilot’s certificate with appropriate type, class, and endorsements.
2) Current and appropriate class medical certificate.
3) When corrective lenses are required by the pilot to meet FAA medical certification, a spare set readily available is required for international operations (suggested for all other operations).
4) Company identification badge.
5) Photo identification must be one of the following:
a. Driver’s license issued by a State, the District of Columbia, or territory or possession of the United States;
b. Government identification card issued by the Federal government, a State, the District of Columbia, or a territory or possession of the United States.
c. U.S. Armed Forces’ Identification card; • Official passport;
d. Credential that authorizes unescorted access to a security identification display area (SIDA badge).
6) FCC Restricted Radiotelephone Operator Permit (only required for International Operations).
7) Valid passport (only required for International Operations). 8) A flashlight in good working condition. 9) Headset in good working condition.
10) A flight bag suitable for carrying, protecting, and securing their EFB as outlined in Electronic Flight Bag (EFB) Program Manual SP 3105.
11) Current Jeppesen Airway Manual, FOM, and SOPM.
FO – What additional documents/equipment are crewmembers required to possess when reporting for duty for an international flight?
Flash light, headset, EFB with cover and extra battery.

Lost or Misplaced Airman and Medical Certificates
1) SkyWest may issue a 72-hour temporary proof of airman or medical certificate under Exemption 11152.
a) The pilot contacts Crew Support and requests issuance of the required proof of certificate. Crew Support provides the pilot a copy of the Crew Qualification as proof of certification. This is used to meet the requirements of 14 CFR Part 121.383 and “Issued under the authority of Exemption No. 11152, as amended.”
2) Replacement of a lost or misplaced airman or medical certificate
a) In the case of a lost or misplaced airman or medical certificate, the pilot must apply to the FAA for a replacement within 72 hours. This is done toll-free to Oklahoma City (866.878.2498) for pilot certificates and 405.954.4821 for medicals (office hours: Monday - Friday, 0800-1600 CST) or online at http://www.faa.gov/. In both cases, the crewmember must request and receive, in writing, a “TEMPORARY AUTHORITY TO EXERCISE CERTIFICATE PRIVILEGES.” The pilot then must update their pilot data as specified above. The written temporary authority to exercise certificate privileges must be carried in place of the lost certificate and expires 60 days after issuance.

B. SkyWest pilots are not required participants in a Hearing Conservation Program; however, pilots are encouraged to wear hearing protection when on the ramp area.
FO – Regarding report for duty requirements and the EFB, what does good working order mean and what is the minimum battery charge to begin a flight segment?

Must report with EFB in good working order. The minimum EFB battery charge to begin a FDP is 80%.
Each pilot must report with the Company approved backup battery charged to 100%.
In the event the backup battery becomes unavailable, ensure the EFB has sufficient battery power based on the dispatch or flight release.
Regarding report for duty requirements and the EFB, what does good working order mean and what is the minimum battery charge to begin a flight segment?
Known condition of not working properly.

Preflight the securing device and its mounting bracket for any cracks or breaks, as well as the EFB for any damage or cracks on the screen.
Power on the EFB. Ensure there is sufficient battery to power the EFB for the duration of the flight leg as required from the release.
Pilots shall limit active applications during critical phases of flight to: • Jeppesen FliteDeck Pro • Company Manuals • Each pilot must report with the Company approved backup battery charged to 100%.
In the event the backup battery becomes unavailable, ensure the EFB has sufficient battery power based on the dispatch or flight release.
FO – What are the currency requirements for the Jeppesen Flight Deck Pro and Company Manuals applications?
1 When the “Today” marker line falls within the green of the bar graphic and there are no updates which are scheduled within the Flight Duty Period window, the data in use by FD-Pro is current and a download is not required for the Flight Duty Period.
2. Company Manuals: synced within four calendar days.

Each pilot must ensure that Jeppesen FliteDeck Pro (FD-Pro), Company Manuals, and FliteView applications are installed and each application has data current for the duty period assignment. Current data is identified for each application as:
1) FD-Pro may show updates available and still be considered current by referencing the “Updates” screen within the App Settings of the FD-Pro application.
a) The graphical screen depicts the three databases which comprise the FD-Pro data and projects when new data will be available.
b) When the “Today” marker line falls within the green of the bar graphic and there are no updates which are scheduled within the Flight Duty Period window, the data in use by FD-Pro is current and a download is not required for the Flight Duty Period.
2) Company Manuals data must have been synced and downloaded within the preceding four calendar days to be considered current for the current day’s operation. The last sync date and time is depicted on the Company Manuals application, lower left screen.
- CA – When reporting for duty with flying on that day, must you be in uniform?
YES, UNLESS APPROVAL FROM MANAGER ON DUTY.
