Are unable to tolerate anticoagulation.
Have difficulty with vascular access.
Have chronic infections or are unstable.
Have chronic diseases (diabetes mellitus, heart failure, severe hypertension).

Hardening of renal arteries, most often due to prolonged HTN and diabetes. Major cause of CKD.
