Increase in serum creatinine by >/= 0.3mg/dL within 48 hours of last value OR

Increase in serum creatinine to >/= 1.5 times baseline (previous lowest value excluding DOL 0-1) OR 

Urine volume of <1 mL/kg/h  (after 24 hours of life or 72 hours in VLBW) OR 

Serum creatinine that does not decrease to less than 1.0 mg/dL by 2 weeks of age and/or is increasing.
